Публікація Development of a mathematical model for predicting postoperative pain among patients with limb injuries(2017) Georgiyants, M.; Khvysyuk, О.; Boguslavska, N.; Vysotska, O.; Pecherska, A.A mathematical model is devised to predict the probability of development of postoperative pain among patients of young age, operated on in a planned manner for the limb injuries. As the model predictors we selected: the level of pain before operation, determined by the visual analog scale, result of evaluation of cognitive abilities by the Montreal scale and level of the mean blood pressure. The application of the developed model makes it possible to improve quality of providing the patients with anesthesiological assistance. The results obtained might be used in the development of information decision support system for a physician-anaesthesiologist for the objectification and automation of the process for determining the probability of development of postoperative pain syndrome. The introduction of such a system into clinical practice will make it possible to reduce the load on the medical staff and decrease the amount of anaesthetising preparations for patients, whose value of the level of pain before operation, determined by the visual analog scale after the operation, does not exceed 3 points, as well as to conduct more adequate analgesia among patients with a higher value of this indicator. Розроблено математичну модель прогнозування ймовірності розвитку післяопераційного болю у пацієнтів молодого віку, оперованих в плановому порядку з приводу травм кінцівок. В якості предикторів моделі обрані: рівень болю до операції, визначений за візуальною аналоговою шкалою, результат оцінки когнітивних здібностей за Монреальською шкалою та рівень середнього артеріального тиску. Разработана математическая модель прогнозирования вероятности развития послеоперационной боли у пациентов молодого возраста, оперированных в плановом порядке по поводу травм конечностей. The application of the constructed model has been demonstrated in order to identify the structure of a cluster in digital photographs. The character of bioproductive processes that define the risk of accumulation of toxic microorganisms is determined by a series of parameters that can be measured remotely using aerospace methods (taking photographs). The proposed model, based on a digital image, makes it possible to restore the spatialdynamic pattern of clusters by determining the state of bioproductive processes in different parts of the cluster. A new approach to determination of imbalance of metabolic processes was proposed. It is based on combining of the flow cultivator and the Lotka-Volterra models. Application of the model of flow cultivator provides an opportunity for objective assessment of initial conditions of behavior of the system acting as a human body and enables time matching of the model basic parameters. Initial conditions for modeling metabolic processes were determined using the Lotka-Volterra model. Use of these conditions makes it possible to determine stability of metabolic processes taking into account individual values of waist and hip circumference, weight, heart rate, age, systolic and diastolic pressure and the calculated Harris-Benedict value. Cross-checking for a possible development of metabolic processes in people with normal and impaired metabolism has shown that the developed approach can be used in medical institutions for the early diagnosis of metabolic disorders. To establish balance of the metabolic processes that are characteristic of people without and with metabolic disorders, retrospective data of examination of 155 young people who were under observation for several years at Kharkiv municipal clinical hospital No. 11, Ukraine, have been analyzed. The proposed approach is the basis of mathematical support of the medical diagnostic information system for detecting imbalance of metabolic processes which is developed currently. Application of such a system in the future will make it possible to study in detail the influence of various gender, territorial and age characteristics on the balance of metabolic processes in the human body.Публікація Development of the descriptive binary model and its application for identification of clumps of toxic cyanobacteria(2017) Nosov, K.; Zholtkevych, G.; Georgiyants, M.; Vysotska, O.; Balym, Yu.; Porvan, A.In the paper, a descriptive model of system dynamics for binary data is presented. Binary or dichotomous data are widely spread across various fields of research - in decision making and data mining, marketing, solving of many natural, social and technical problems. The initial data for building the model is a set of states of an autonomous dynamical system with components taking binary values. At the same time, the time order of the states is permissible. The following objectives were stated: to identify the relationships between the components of the system defining its dynamics; on the basis of the identified dynamics, to recover the true order of the system states; to apply the developed model to the problem of visual identification of cyanobacteria in water areas using digital photography. To solve the problem, we used a mathematical model that enables to describe the relationships between components and transitions between the system states at a simple-forunderstanding level. The principle of parsimony underlies the model. According to this principle, the most appropriate model is described by the simplest relations in the sense defined in the work. As the case study, the problem of recognizing clumps of cyanobacteria from digital satellite imagery was considered. This is a complex, practically important problem that does not have a satisfactory experimental and theoretical solution at the moment. Applying system approaches to the measured colorimetric parameters of digital photography, we developed the index for identification of such clumps. This index uses the parameters of the digital RGB model of (various parts of) an image and allows us to reveal clumps of cyanobacteria on digital images obtained by aerospace methods. Additionally, digital photography can be performed in the conditions of insufficient visibility (due to precipitation, fog, and other factors), for imitation of which in the case study the original image was distorted by the digital noise. The studied model can find useful applications in the areas requiring binary dynamical data insights. The investigated plant communities can be used as natural bioplato for the elimination of biosafety threats to water consumption. The relevance of these studies is determined by the increased need for expanding the arsenal of methods for remote diagnosis of the states of natural systems that are important for biosafety provision. In particular - to ensure biosafety when using natural feed resources by waterfowls, which are a potential reservoir of bird flu. The similarity in the dynamics of the colorimetric parameters of phytocenoses and the Margalef's succession model makes it possible to implement a new approach to the generation of productive working hypotheses for the development of remote methods for determining the state of bioproduction processes in natural bioplato. The proposed approach is based on the use of the class of mathematical models, which is called the discrete models of dynamic systems. Based on the structure of the correlations between the colorimetric components of space photographs of the plavni in the mouth of the river Danube, a description of the structure of the intercomponent and intracomponent relations of the massifs of semi-submerged higher aquatic plants has been obtained. The resulting structure of intercomponent relations allowed us to construct idealized trajectories reflecting the dynamic changes of the system. A unique constant inverse relationship between the parameter reflecting the amount of green chlorophyll pigment affecting the level of photosynthetic production and the parameter reflecting the amount of orange-red pigments in each of the possible matrices of the ratios of colorimetric parameters has been revealed. As a result of analysis of the dynamic aspects of the RGB model, the structure of the system color parameter is shown, which is the mean square deviation of the spread in the degree of alignment of parameter values reflecting the amount of green and orange-red pigments. Представлені результати моделювання межкомпонентних відносин колорометричних параметрів природних біоплато. В якості вихідних даних використані космічні знімки плавнів в дельті Дунаю. Запропоновано використовувати системний колорометрический параметр, що відображає розмах значень ступеня вирівняності значень колорометричних параметрів. Виділені ознаки дозволяють дистанційно діагностувати стан біопродукційних процесів природних біоплато. Представлены результаты моделирования межкомпонентных отношений колорометрических параметров естественных биоплато. В качестве исходных данных использованы космические снимки плавней в дельте Дуная. Предложено использовать системный колорометрический параметр, отражающий размах значений степени выровненности значений колорометрических параметров. Для моделювання використана дискретна динамічна модель. Структурно-параметрична ідентифікація моделі здійснювалася на основі цифрових знімків тварин і рослинних спільнот у місцях їх мешкання. Визначені системні колорометричні параметри, використання яких дозволило підвищити контрастність силуетів тварин на фоні фітоценозу. Це підвищення контрастності сприяє демаскуванню тварин на місцевості, що є необхідним при проведенні заходів щодо усунення загроз біобезпеки, пов'язаних зі сказом. Представлены результаты математического моделирования защитной окраски лисиц с целью выявления животных на местности. Для моделирования использована дискретная динамическая модель. Структурно-параметрическая идентификация модели осуществлялась на основе цифровых снимков животных и растительных сообществ в местах их проживания. Определены системные колорометрические параметры, использование которых позволило повысить контрастность силуэтов животных на фоне фитоценоза. Это повышение контрастности способствует демаскировке животных на местности, что необходимо при проведении мероприятий по элиминации угроз биобезопасности, связанных с бешенствомПублікація Optimization of postoperative analgesia after adenoidectomy in children(2017) Georgiyants, M.; Pushkar, M.; Vysotska, O.It is known that pharyngalgia is very common after tonsillectomy. It should be emphasized that the intensity of pain after adenoidectomy in children is not less important than after adenotonsillectomy. Despite the availability of standardized pain assessment scales and existing postoperative analgesia recommendations, unresolved postoperative pain still occurs in children. The research included 117 children with an average age of 7.5 ± 0.4 years, who underwent adenoidectomy at the Department of Anesthesiology and Intensive Care of "Regional Children’s Clinical Hospital" of Kharkov city in 2014. Depending on the method of general anesthesia, patients were divided into 3 groups: group I (n = 41) those who received propofol in combination with fentanyl; group II (n = 40) those who received sevoflurane in combination with fentanyl; group III (n = 36) those who received thiopental sodium combined with fentanyl. We monitored the heart rate, systolic blood pressure, diastolic blood pressure, mean arterial pressure, BIS-index, heart rate variability, respiration rate, and SpO2. We determined the levels of cortisol and insulin in the blood serum, glucose level, the ratio of cortisol/insulin was calculated. Assessment of the efficiency of postoperative analgesia was performed using the Wong-Baker FACES Pain Scale («Faces») and the Oucher Scale. The result of analysis of the intensity of postoperative pain determined that at the 1st hour after the operation by the «Faces» and Oucher scales, it was significantly higher in patients of group II compared with the patients of group I and group III. The morning after the operation there was no significant difference in the pain scales, and the number of scale points showed that children from all groups did not have pain. According to the data of ANOVA it was determined that only the patients in group I with indicator ΔBIS-index «intubation – traumatic moment of operation» experienced postoperative pain intensity on the «Faces» scale. A very strong correlation between «cortisol – BIS index» was observed during the traumatic moment of operation and unidirectional positive correlations were seen both between ΔBIS-index «intubation-the traumatic moment of operation» and between the level of cortisolemia (Δcortisol before surgery – the traumatic moment of operation, Δcortisol extubation – the 1st day after the surgery and Δcortisol before surgery – the 1st day after the surgery) and the intensity of postoperative pain by the «Faces» and Oucher scales. The around-the-clock prescribed administration of ibuprofen at dose 10 mg/kg after adenoidectomy provided effective postoperative analgesia. At the 1st hour after the operation lower pain intensity was revealed in patients using propofol in combination with fentanyl by both pain scales. We believe that propofol is able to influence the level of cortisol and assume that due to minimal changes in the level of cortisol during the perioperative period, propofol can reduce the intensity of postoperative pain.
